(2)b Department of Interventional Radiology and Neuroradiology, Medical University of Lublin, Lublin, Poland. We reported the case of a patient with Wernicke-Korsakoff syndrome (WKs) as an early clinical manifestation of sporadic Creutzfeld-Jakob disease (sCJD). Korsakoff's Syndrome Korsakoff's syndrome, or psychosis, the chronic form of the Wernicke–Korsakoff syndrome, is characterized primarily by an amnestic dementia, that is, a profound disorder of memory with relative preservation of cognitive abilities per se. The Korsakoff syndrome is defined as a disproportionate impairment in memory, relative to other aspects of cognitive function, resulting from a nutritional (thiamine) depletion. Wernicke-Korsakoff syndrome is a common complication of a thiamine deficiency that is primarily seen with alcoholics.

The Wernicke-Korsakoff syndrome are two separate but related stages of the Elliott L. Mancall, in Neurology and General Medicine (Fourth Edition), 2008 Korsakoff's Syndrome. Korsakoff's syndrome, or psychosis, the chronic form of the Wernicke–Korsakoff syndrome, is characterized primarily by an amnestic dementia, that is, a profound disorder of memory with relative preservation of cognitive abilities per se. Alcohol is linked to the development of Wernicke-Korsakoff syndrome, a brain disorder resulting from thiamine (vitamin B1) deficiency. Wernicke-Korsakoff syndrome is actually a two-stage brain disorder in which Karsakoff syndrome (also known as Korsakoff psychosis) develops due to permanent brain damage as symptoms of Wernicke encephalopathy wane. Korsakoff syndrome (KS) is an amnestic disorder caused by thiamine (vitamin B 1) deficiency typically associated with prolonged use of alcohol. The syndrome and psychosis are named after Sergei Korsakoff, the Russian neuropsychiatrist who discovered it during the late 19th century.
Parts of the brain may be damaged as a result of this deficiency, causing increased difficulty with memory, movement, vision and coordination. Korsakoff syndrome (KS) refers to a chronic neurologic condition that usually occurs as a consequence of WE. In 1881, Carl Wernicke described an acute encephalopathy characterized by mental confusion, ophthalmoplegia, and gait ataxia and associated it with autopsy findings of punctate hemorrhages around the third and fourth ventricles and the aqueduct.

Note that this is on a background of global cerebral atrophy with CSF spaces more prominent than would normally be expected for a patient of 45 years old. Wernicke encephalopathy, also referred as Wernicke-Korsakoff syndrome, is a form of thiamine (vitamin B 1) deficiency, and is typically seen in alcoholics.. On imaging, it is commonly seen on MRI as areas of symmetrical increased T2/FLAIR signal involving the mammillary bodies, dorsomedial thalami, tectal plate, periaqueductal area and/or around the third ventricle. Quantitative analyses were performed on computer tomography (CT) scans from 7 patients with Korsakoff's syndrome, 7 age-matched alcoholic subjects, and 7 age-matched healthy control subjects.

Korsakoff syndrome affects the ability to learn new information, remember recent events and causes long-term memory gaps. Memory challenges can be the most prominent symptom, while other thinking and social skills might remain relatively unaffected.

The patient subsequently developed encephalopathy, at which point thiamine deficiency was suspected and thiamine supplementation initiated. Wernicke’s encephalopathy and Korsakoff’s psychosis are the acute and chronic phases, respectively, of the same disease. WKS is caused by a deficiency in the B vitamin thiamine.
